When you use the contact form on this site, we receive the name, email address, phone number (optional), and message you type. If you call, text, or email us, we receive what you send.
Only to reply to you, prepare a quote, and provide courier service you request. We do not sell your information and we do not send marketing email you did not ask for.
The contact form is delivered by a third-party form service (FormSubmit) to our email inbox, which is hosted by Google Workspace. Those services handle your message so we can receive it.
Do not include patient names, medical details, or other protected health information in the form, by email, or by text. We will arrange a secure way to handle anything sensitive once we are working together.
This site does not use advertising cookies. If we add basic visitor statistics later, we will update this page.
